Remedial Action Plan

Service Overview

A Remedial Action Plan is a structured, regulator-aligned improvement programme designed for providers who have failed one or more sections of their CQC inspection under the Single Assessment Framework (SAF).
Where the CQC identifies breaches or “Requires Improvement”/“Inadequate” ratings in any of the five areas — Safe, Effective, Caring, Responsive, Well-Led — the provider must produce a clear, measurable, and time-bound action plan demonstrating how they will restore compliance.

Scope of Service

Our RAP service delivers that plan, engineered to CQC standards, backed by evidence, and structured to turn around your governance, quality, and operational performance.
Who This Service Is For
This service is designed exclusively for providers who:

  • Have already been inspected
  • Have received a negative rating or breaches in one or more SAF quality categories
  • Are required to submit an action plan to CQC
  • Need urgent, structured, and accountable compliance recovery

If you received a rating of:

  • Requires Improvement
  • Inadequate
  • A breach notice under any Regulation
  • A Warning Notice
  • Or flagged issues under a specific SAF domain

— our RAP service is designed for you.

What We Offer (SAF-Aligned Compliance Recovery)

We produce a CQC-ready Remedial Action Plan that includes:

1. Full Review of CQC Findings
We analyse your inspection report line-by-line, mapping each concern to:

  • The relevant Quality Statement
  • The Evidence Category
  • The Regulation breached or at risk
  •  The operational failure that triggered the rating

2. Root Cause Analysis
We identify why non-compliance occurred:

  • Governance gaps
  • Leadership issues
  • Process failures
  • Documentation deficiencies
  • Workforce competency gaps
  • Cultural/behavioural factors

3. Action Plan Development (One Plan per Failed Area)
Each Key Line of Enquiry (KLOE/SAF domain) receives:

  • Specific corrective actions
  • Required evidence
  • Responsible person
  • Timescale for completion
  • Governance oversight structure
  • KPIs and monitoring tools

4. SAF Evidence Mapping
For each failing area, we detail:

  • What evidence CQC requires
  • What you currently lack
  • What needs to be produced
  • How it will be monitored

5. Governance & Culture Turnaround
Depending on the failing area, actions may include:

  • New governance meetings & reporting structure
  • Updated policies, registers, and audit tools
  • Workforce competency plans
  • Leadership intervention & oversight
  • Staff culture and behavioural improvements

New incident, complaints, or risk management systems
6. Inspection-Ready Output
You receive:

  • A CQC-compliant RAP document
  • Evidence appendix (if required)

  • Monitoring template

This is structured so CQC inspectors can easily review your progress at re-inspection or follow-up.
