Ofsted Safeguarding Audit & Incident Review Service
Service Overview
Safeguarding failures are the fastest route to enforcement action, poor inspection outcomes, and regulatory loss of confidence.
When an incident occurs, Ofsted expects providers to demonstrate strong oversight, independent scrutiny, and clear learning.
Our Safeguarding Audit & Incident Review service delivers exactly that — rigorous analysis, impartial judgment, and actionable improvements.
Scope of Service
We provide a full independent safeguarding governance review, including:
- Comprehensive Safeguarding Audit against Children’s Homes Regulations, Quality Standards, Working Together, SCCIF expectations, and local multi-agency procedures.
- Incident Review / Case File Review to establish what happened, what should have happened, and where practice fell short.
- Root Cause Analysis (RCA) identifying systemic and human factors contributing to the incident.
- Recording & Reporting Quality Review, including chronology audits, decision-making checks, and escalation mapping.
- Leadership & Management Evaluation focusing on oversight, professional curiosity, risk management, and culture.
- Action Plan & Improvement Roadmap with short-, medium- and long-term recommendations.
- Optional Multi-Agency Liaison to ensure alignment with placing authorities and safeguarding partners.
This gives providers a defensible, professional, and compliance-driven safeguarding position that withstands Ofsted scrutiny.
